Hart Dental has been treating and caring for patients in and around the Unley area for over a decade.

We pride ourselves on our family treatment programs and are committed to providing you with the finest in dental care and cosmetic dentistry procedures.

Financial Policies

Payment Policy:

Hart Dental are preferred providers for NIB and CBHS but accept all health funds. We are also members of smile.com.au and accept Government schemes such as GDS and EDS forms as well as DVA Gold cards.

Payment is due on the day, however we do have payment plan services available for large treatment plans. These include in-house payment plans, Afterpay, DentiCare and the National Dental Plan.

Reservation fees:

Purpose: A reservation fee of $250 is required for appointments over 60 minutes to secure your appointment and assist in managing our schedule. This fee reflects your commitment and is applied directly to the cost of your appointment.

Payment:
– 
Fees can be paid by cash or card at the time of scheduling or at least 5 says before the appointment.
The fee will be deducted from the total treatment cost on the day of service.
– The fee may be forfeited if the appointment is missed or changed with insufficient notice.

Cancellations:
To avoid forfeiting the reservation fee, please provide at least 3 working days’ notice if you need to cancel or reschedule your appointment.
If notice is less than 3 days, the fee may be forfeited at the dentist’s discretion.
Rescheduling:
If sufficient notice is given, the deposit will transfer to the new appointment with adjustments made as necessary.
– The reservation fee is refundable or transferable if the appointment is cancelled or rescheduled with sufficient notice.

Exceptions: Fees may be waived for unavoidable circumstances such as illness or accidents. The reservation fee is non-refundable if the appointment is missed or cancelled without sufficient notice.

Policy Communication: We ensure that the terms of the deposit and reservation fee are clearly communicated to you. If you have any questions about our financial policy, please ask our friendly staff.

Purpose of Information Collection:

We collect personal and health information to ensure quality care. This includes:
– Name, date of birth, contact details
– Emergency contact details
– Private Health Insurance and Medicare Details
– Information on Workers Compensation or Accident Claims
– Oral and General health history, including medical history
– Photos for treatment and advertising purposes.

Right to Refuse Disclosure:

You have the right to refuse to provide certain information. However, not providing relevant information may impact the acre and treatment we can offer.

Use of Your Information:

We can use your information for:
– Assessing your oral health and providing treatments
– Communicating with other health care providers involved in your care
– Processing payments and insurance claims
– Updating records and addressing any complaints
– Supporting staff training, quality improvement and research (using de-identified data)

We will seek additional consent for any uses of your information beyond those described, unless required by law.

Disclosure of Your Information:

Your information may be shared with:
– Our Dentists and Practice Staff
– External dental care providers (eg. specialists, laboratories)
– Emergency contacts
– Government and regulatory bodies (eg. Medicare)
